COVID-19 Waiver of Liability

Due to the pandemic of the novel Coronavirus (COVID-19), Paws Up Professional Dog Obedience Training, LLC is taking precautions to protect you, your dog and our community. To this extent, Paws Up Professional Dog Obedience Training, LLC will be following the Center for Disease Control (CDC) and the PA Department of Health and Human Services guidance with regard to social distancing practices and sanitation.

Symptoms of COVID-19 include:

Fever or chills
Fatigue
Cough
Shortness of breath or difficulty breathing
Muscle or body aches
Headache
New loss of taste or smell
Sore throat
Congestion or runny nose
Nausea or vomiting
Diarrhea
CDC's current list of symptoms reported

I agree to the following by signing this form:

I, nor members of my household, have not experienced any of the symptoms listed above within the last 14 days.

I, now members of my household, to the best of our knowledge have not been exposed to someone with a suspected and/or confirmed case of the Coronavirus (COVID-19).

I, nor members of my household, have not been tested and found positive for Coronavirus (COVID-19) within the last 14 days nor are awaiting test results.

I agree that should I or a member of my household become ill with COVID-19 symptoms I will immediately inform my instructor.

I agree that if I or a member of my household becomes aware of being exposed to a confirmed or suspected case of COVID-19 that I will notify my instructor immediately.

I agree to follow all Paws Up Professional Dog Obedience Training, LLC rules and requirements to reduce any exposure and possibility of contracting or spreading the virus. I agree that I am attending these dog training classes/events at my own risk and assume all risk and full responsibility for my own health, wellbeing, and safety. I agree that Paws Up Professional Dog Obedience Training, LLC, the owner Mindy L. Columbus are in no way liable for any present or future COVID-19 exposure incurred at any time by any person, in attendance or not in attendance, during or after this class or event. Any liability to anyone for any incident involving myself, the location, or those pets or actions of other participants and spectators will be my sole responsibility and I assume all financial liability and will also assume any and all financial costs associated with my actions.

I understand according to the CDC, at this time, there is no evidence that animals play a significant role in spreading the virus that causes COVID-19. Based on the limited information available to date, the risk of animals spreading COVID-19 to people is considered to be low. The CDC is still learning about this virus, but it appears that it can spread from people to animals in some situations. I understand that choosing to work with my pet in a private or group class setting/event is not without risk.

Paws Up Professional Dog Obedience Training, LLC is requiring the following procedures for all classes/events at South Park and DeLuca's Canine Country Club:

Wearing of masks is required for individuals who are not fully vaccinated.
All attendees will maintain social distancing of at least 6 feet.

Paws Up Professional Dog Obedience Training, LLC will follow Centers for Disease Control (CDC), PA State Department of Health Guidelines plus any additional guidelines recommended per Allegheny County Parks and DeLuca's Canine Country Club.

I have read and agree to the statements of this COVID-19 liability waiver.
